As a Team We Can Make a Difference

All staff at the Whitewater Bromley Community Health Centre work as a team to deliver the best possible service to our clients and community. Our primary care providers and allied health staff work as an interdisciplinary team to ensure that each client is directed to the right provider at the right time for the care they need.

Members of our Health Team include:

Our Medical Secretaries are the first point of contact for clients and community members. They schedule appointments and referrals for clients and offer valuable information regarding Health Centre programs and services.

Our Doctors are family physicians who have an expert knowledge of a wide range of common health problems in all age groups and of less common but life threatening and treatable emergencies. They see patients with chronic diseases, acute disorders, complex health conditions and emotional difficulties. They may provide palliative care to people with terminal illnesses. They provide care for people here at the health centre, and occasionally in their home.

Our Nurse Practitioners (NPs) are advanced practice nurses who work in roles similar to family doctors. Clients at our centre receive their primary care from either a Nurse Practitioner or a Doctor. The NPs offer a unique blend of nursing, medical knowledge and skills. They can perform routine procedures such as health exams and well-baby visits, order tests and medications, and care for clients with either chronic (diabetes or high blood pressure) or acute (bladder infections, earaches or sprains and cuts) conditions.

Our Nurses include Registered Nurses and Registered Practical Nurses. They provide information to the client about their health concern and how to live a healthy lifestyle, monitor the client’s condition and record their response to treatments and medications. They meet with clients at the Health Centre, by telephone, in their homes as needed.

Our Chronic Disease Nurse focuses on chronic disease prevention and management.  This position conducts and/or facilitates the co-ordination, planning, development, implementation and evaluation of chronic disease programs and activities for residents of our community.

Our Social Work Counsellor offers counselling and support services to our primary care clients, as well as group programs to residents of our community . Counselling Services are offered as a means to help people develop skills to address the challenges they face and to build confidence in their own inner strengths and resources.

Our Respiratory Educator works with other Health Team members to assist clients with breathing problems. They develop treatment plans and assess options to help treat people with pneumonia, lung cancer, asthma, chronic obstructive pulmonary disease (COPD), smoking cessation, recent lung surgery, and other medical conditions that affect breathing.

Our Community Health Promoter works with individuals, organizations and coalitions to address community needs. Currently our work focuses on: mental health and wellness, physical activity, families with young children, healthy eating, volunteer engagement, and community capacity building related to poverty, food security, chronic disease, mental health and addictions.

Our Chiropodist specializes in the study, management and prevention of disease and disorders of the foot. Registered clients of the health centre who need specialized foot care can be referred to our chiropodist by their physician or nurse practitioner.
